Guardian Angel Home Care | San Jose, California Pay: $45–$50 per hour, depending on experience Schedule: Monday–Friday | 11:00 AM – 5:00 PM Hours: 30 hours per week Start Date: October 19, 2026 Location: San Jose, California Position Type: Part-Time / Private Duty Nursing ABOUT THE ROLE Guardian Angel Home Care is seeking a compassionate, dependable, and experienced Licensed Vocational Nurse (LVN) to provide private duty nursing care for a patient in the San Jose, California area. This position offers a consistent weekday schedule, working Monday through Friday from 11:00 AM to 5:00 PM, providing dedicated one-on-one nursing care in the comfort of the patient's home. We are looking for an LVN who is passionate about patient care, comfortable working independently, and committed to building a positive relationship with the patient and their family. WHAT WE OFFER * Competitive hourly pay: $45–$50, depending on experience * Consistent Monday–Friday schedule * 11:00 AM – 5:00 PM (30 hours per week) * No scheduled weekends * One-on-one private duty patient care * Supportive clinical leadership and administrative team * Electronic documentation through Homecare Homebase (HCHB) * Opportunity to provide personalized, meaningful patient care RESPONSIBILITIES * Provide dedicated one-on-one skilled nursing care to a patient in their home. * Monitor the patient's condition, vital signs, and overall health status. * Administer medications and treatments as prescribed and within the LVN scope of practice. * Assist with activities of daily living and patient care needs as appropriate. * Observe and promptly report changes in the patient's condition to the supervising RN and physician. * Maintain accurate and timely clinical documentation. * Communicate professionally with the patient, family, and interdisciplinary care team. * Follow the established plan of care and all applicable agency and state regulations. * Maintain a safe, comfortable, and supportive environment for the patient. REQUIREMENTS * Active California Licensed Vocational Nurse (LVN) license in good standing * Graduate of an accredited vocational nursing program * Current BLS/CPR certification * Previous nursing experience * Strong communication and patient care skills * Ability to work independently in a private home setting * Dependable and available for the assigned weekday schedule PREFERRED QUALIFICATIONS * Previous private duty nursing or home health experience * Experience providing one-on-one patient care * Familiarity with electronic medical documentation systems * Strong medication administration and patient assessment skills WHY JOIN GUARDIAN ANGEL HOME CARE?
