Registered Nurse Outpatient Clinic

Grace Community Care & Homes is developing its Medical and Preventive Care department and seeks a Registered Nurse Outpatient Clinic. The role will contribute to person-centered services, reliable operations and accurate documentation. Core responsibilities Provide nursing assessment, patient education, care coordination and clinical support; maintain accurate documentation and escalate changes in condition. Work collaboratively with department leadership, intake, credentialing, billing and quality teams. Maintain confidentiality, respect participant choice, communicate service gaps promptly, and follow role-specific policies and professional scope. Participate in orientation, competency review and quality improvement. Program readiness This is a planned role. Start date and duties are contingent on applicable program approvals, funding, credentialing and staffing readiness. Services may begin only within the approved scope. Location and schedule New Jersey, with administrative coordination through Grace and on-site or community assignments as applicable. Part-time; assignment and weekly schedule discussed during hiring. Clinical services begin only at approved locations and within authorized scope. Active NJ RN license; outpatient nursing and patient-education skills; current role-appropriate life-support training.
