Responsibilities Provide skilled nursing care and administer prescribed medications and treatments. Perform G-tube/PEG care, tracheostomy care, and suctioning as required. Monitor patient conditions and report changes promptly. Provide ventilator support when required and appropriately trained. Educate patients and caregivers and maintain accurate documentation. Qualifications Active New York State LPN or RN license. Minimum of 6 months of home care experience. Current CPR/BLS certification. Strong clinical and communication skills.
