Job Description: Lehigh Valley Companion Care Homes is seeking a compassionate and dedicated Registered Nurse (RN) to join our team on a PRN basis. The ideal candidate will deliver high-quality in-home and community-based care to clients, ensuring their health and well-being needs are met in a comfortable and nurturing environment. Responsibilities: * Provide skilled nursing care to patients in their homes and in the community setting. * Administer medications and monitor treatment plans as prescribed. * Perform health assessments, including vital signs, and document findings. * Assist with personal care tasks, as needed. * Support patients with mobility and physical therapy exercises to enhance their overall well-being. * Ensure patient safety and comfort through proper care and supervision. * Coordinate with family members and healthcare providers to ensure optimal care and support. * Respond to medical emergencies as needed and provide first aid as required. * Document all care provided and maintain accurate patient records to ensure comprehensive and up-to-date documentation.
