Your responsibilities will include: Conducting comprehensive patient assessments and reassessments Developing, implementing, and updating individualized Plans of Care Providing skilled nursing interventions based on each patient's clinical needs Completing accurate and timely OASIS assessments and clinical documentation Educating patients and families regarding disease processes, medications, treatments, and plan of care Monitoring patient progress and communicating changes in condition appropriately Collaborating with physicians and the interdisciplinary care team Promoting patient safety, quality outcomes, and continuity of care Providing compassionate, professional care that allows patients to remain safely at home What We're Looking For We are looking for a nurse who is confident working independently, clinically strong, organized, and committed to providing high-quality care.
