Job Overview Ebenezer Integrated Care & Rehab (EICR) serves seniors with long-term, transitional, and dementia care needs in the heart of downtown St. Paul. As part of Fairview's Community Health and Wellness Hub, EICR is committed to delivering innovative, collaborative care. The Fairview Community Health and Wellness Hub opened in 2022 to deliver a variety of services to the community—from primary and mental health care to enrichment options for seniors, food access programs, and community gathering spaces. The Hub brings Fairview services together with local organizations—including Minnesota Community Care and Sanneh Foundation—to make it easier for people to access what they need to be well. We are seeking a Licenesed Practical Nurse - LPN for our newest Skilled Nursing Community (Transitional Care units, Long Term Care) in downtown St. Paul, Minnesota! This LPN role assists in meeting the residents' physical, emotional and psycho-social needs while adhering to all facility, state and federal guidelines, Ebenezer Standards and Policies. This schedule includes; 48 hours per two weeks Day shift Block Schedule: Week 1: Thursday, Friday Week 2: Monday, Tuesday, Saturday, Sunday Memory Care Unit Every other weekend and holiday rotation Duties: Provides skilled nursing care such as; implementing practitioners’ orders, ordering and assisting with medications and supplies, operates medical equipment, and charting practices Lifts and transfer residents as needed, Initiates emergency treatment as needed Monitors work and performance of unlicensed staff Updates RN with current observation and completes documentation of resident health status Communicates changes in residents conditions to relevant individuals May support RN by completing resident assessments, service agreements and care plans Required: MN LPN licensure AHA BLS for Healthcare Providers or Red Cross CPR for Professional Rescuers Able to work independently Good communication and interpersonal skills Preferred: Previous healthcare experience Comfortable using computers, hand held devices, phones and fax machine Benefit Overview Ebenezer offers a generous benefits package, including but not limited to medical, dental, vision plans, life insurance, short-term and long-term disability insurance, PTO and Sick and Safe Time, tuition reimbursement, retirement, early access to earned wages, and more!
