GIM- Ambulatory Breast Diagnosis Clinic The Licensed Practical Nurse (LPN) –provides direct patient care and clinical coordination within the high-volume ambulatory Breast Diagnostics Clinic. This role supports timely access to care, ensures clinical readiness for visits, and serves as a key communication liaison among patients, providers, and the multidisciplinary care team. This position plays a critical role in managing complex clinical workflows within a fast-paced ambulatory setting. The LPN supports both patients and care teams through proactive coordination, clear communication, and a commitment to patient-centered care. Key Responsibilities: * Conduct patient visits, including review of health history, medication reconciliation, and vital sign collection (e.g., blood pressure), in accordance with established clinical protocols. * Complete pre-visit and pre-call chart reviews to ensure clinical readiness, accurate documentation, and appropriate coordination of care. * Perform telephone encounters to assess patient needs, review clinical information, and coordinate care. * Collaborate daily with providers, registered nurses, desk operations specialists, patient appointment services, interpreters, and medical administrative staff to support efficient patient flow and access. * Serve as a resource to the care team by responding to questions, clarifying plans, and facilitating next steps in the patient care continuum. * Coordinate collection of outside medical records and diagnostic information to support comprehensive patient evaluation. * Promote effective communication and continuity of care across the multidisciplinary team to enhance patient experience and outcomes. * Practice in accordance with LPN scope of licensure, Mayo Clinic policies, and professional nursing standards. The licensed practical nurse (LPN) provides nursing care to assigned patients under the direct supervision of the registered nurse (RN) or other assigned supervisor and accepts delegation from the RN/assigned supervisor in meeting the needs of the patient/family. The LPN collects data through observation and communicates information to assist the RN and/or provider in patient assessment and care planning. The LPN participates as a member of the health care team and accepts delegation from the RN/assigned supervisor and provider in meeting needs of the patient/family. The LPN delivers care in accordance with the patient care plan, policies and procedures of the organization and principles of relationship-based care. The LPN possesses excellent communication skills; is skillful in mentoring and instructing; and may participate on committees or projects, including quality improvement projects. QUALIFICATIONS Graduate of a school of practical nursing or passed LPN Boards after a defined period in a professional nursing program (e.g., some states allow RN students to take the LPN board exam after completing one to two years in the RN program). If graduation did not occur within the last two years, one year of LPN experience working in an applicable setting is required. One year LPN experience within the last five years preferred within the Ambulatory care setting.  Experience working in a team environment. Computer skills required, prior experience with electronic medical record systems preferred. Ability to work daytime hours, flexibility may be required to meet staffing needs. Ability to adapt to unpredictable situations within the work setting. Graduate of an accredited school of practical nursing preferred.  Current LPN license by applicable state requirements. Arizona and Florida: Maintains current Basic Life Support (BLS) for Health Care Providers. Midwest: Maintains current Basic Life Support for Health Care Providers from one of the following programs: American Heart Association or American Red Cross. Additional specialty certification/training as required by the work area.  Positions that are not on campus may not require current Basic Life Support (BLS) competency as determined by the work area.
