Position Title: Nurse, Pilot - 1099 Department: Pilot Reports To: Director of Cardiac Sales, Three Rivers Region Location / Work Environment: Office Environment Worker Classification: 1099 Contractor FLSA Status: Unknown Job Level: Standard Job Purpose The Nurse, Pilot supports ROMTech's Clinician guided at-home rehab and telehealth initiatives through patient outreach, education, care coordination, and clinical support activities conducted in a virtual care environment. This contract-based role contributes to the evaluation and development of innovative healthcare delivery models by engaging patients, monitoring progress, supporting care management activities, and collaborating with interdisciplinary teams. The role helps enhance patient access, engagement, continuity of care, and program effectiveness while maintaining compliance with applicable clinical, regulatory, and documentation standards. Key Responsibilities Essential Duties Conduct telehealth outreach to patients, providing education, follow-up support, and care coordination services. Monitor patient progress and communicate patient needs, concerns, and status updates to appropriate clinical team members. Support patient engagement and adherence to recommended care plans and program requirements. Collaborate with multidisciplinary clinical and operational teams to support established patient care goals. Participate in pilot-program workflows and provide feedback to support service development and process improvement. Accurately document patient interactions and activities in electronic health record systems and other designated platforms. Support Chronic Care Management (CCM) activities when applicable. Maintain compliance with applicable healthcare regulations, patient privacy requirements, documentation standards, and organizational policies. Utilize clinical judgment within the scope of licensure to support patient-centered care delivery. Maintain active licensure and eligibility to provide services across applicable jurisdictions. Other Functions Participate in training activities related to pilot program objectives and service delivery models. Contribute feedback regarding workflow design, patient engagement strategies, and operational improvements. Support quality, compliance, and program evaluation activities as requested. Performs other mutually agreed upon duties consistent with the applicable engagement or contract. Qualifications: JOB QUALIFICATIONS Skills and Abilities Required Strong patient communication and education skills. Excellent organizational and time-management abilities. Ability to work effectively within a multidisciplinary clinical environment. Proficiency with electronic health records (EHR) systems and healthcare technology platforms. Ability to maintain accurate, timely, and compliant clinical documentation. Knowledge of patient privacy, confidentiality, and regulatory requirements. Strong critical-thinking and problem-solving skills. Demonstrated commitment to patient-centered care. Preferred Familiarity with telehealth delivery platforms and virtual care technologies. Experience supporting Chronic Care Management (CCM) programs. Experience in care coordination and patient engagement initiatives. Experience participating in pilot programs, virtual care projects, or healthcare innovation initiatives. Work Experience Requirements Required Prior telehealth and/or care management experience. Experience providing patient support, patient education, or care coordination services. Preferred Experience supporting remote patient monitoring programs. Experience with chronic disease management or rehabilitation-focused patient populations. Experience working in innovative healthcare delivery or pilot-program environments. Education Requirements Required None. Preferred Graduate of an accredited Licensed Practical Nurse (LPN) or Registered Nurse (RN) program. License and Certification Requirements Required Active Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in good standing. Compact nursing licensure with multi-state practice privileges. Preferred Additional certifications related to care management, telehealth, rehabilitation, chronic disease management, or patient education. Physical Demands and Working Conditions Office Environment Ability to sit at a computer terminal for an extended period. Moderate noise (i.e., phone calls, online meetings, computer audio). While performing the duties of this role, the individual may be regularly required to stand, sit, talk, hear, reach, stoop, kneel, and use hands and fingers to operate a computer, telephone, and keyboard. Specific vision abilities required include close vision requirements due to computer work. Must be able to communicate clearly and professionally in both verbal and written formats. Must be able to engage in active listening and express ideas effectively in virtual meetings and patient interactions.
