Home Health Registered Nurse (RN) PRN

Quick Facts Pay: Negotiable based on experience, knowledge, skills, and abilities Location: Inverness and surrounding areas Schedule: PRN, successful candidates will have 2-3 days of availability weekly from 7:00a-4:00p Employment type: W-2 Mileage: Currently $0.60/mile Reports to: Branch Director What You'll Do Assess each new patient's condition, history, and home care needs Build the care plan with the patient and family Update the care plan as the patient's status changes Administer medications and treatments as prescribed Teach patients and families how to manage care at home Coordinate with physicians and send updates at least every 60 days Coordinate with community health partners involved in the patient's care Plan and carry out discharge when the patient is ready Arrange any equipment or services the patient needs Check in on home health aide care every two weeks Qualifications Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting. Current licensure in state and CPR certification. Preferred: Bachelor's degree, with one (1) year of home health care experience. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. This position requires regular travel by personal vehicle between patient homes, the office, and other locations throughout the assigned service area. The clinician must hold a valid driver's license, maintain reliable transportation and current auto insurance, and be able to drive safely for extended periods in varying weather and traffic conditions.
