Registered Nurse (RN) – Home Health (Per Diem) Location: Sacramento County Schedule: Thursday-Monday Compensation: Per-Visit Pay Structure About the Role Sonder Healthcare is seeking an experienced Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their homes throughout Sacramento County. This position offers schedule flexibility, autonomy in the field, and the opportunity to make a meaningful impact within the community. Responsibilities Perform initial and ongoing patient assessments Develop and update individualized plans of care Provide skilled nursing care, treatments, and medication administration Educate patients and caregivers on disease management and self-care Coordinate care with physicians and interdisciplinary team members Maintain accurate and timely clinical documentation Qualifications Active California RN license (required) Minimum of three (3) years nursing experience Home health experience required Current BLS certification (American Heart Association) Valid driver’s license and reliable, insured transportation Strong clinical, documentation, and communication skills Why Join Sonder Healthcare?
