WHAT YOU’LL DO * Deliver compassionate, high-quality care to patients across the lifespan * Partner with providers and care teams to ensure seamless, coordinated treatment * Support patients and families in understanding health needs and next steps * Document care accurately using our EHR system * Participate in care huddles and team-based quality improvement initiatives * Serve as a key member of our Patient-Centered Health Home model What We’re Looking For Required: * RN / LPN / CMA License OR Certification, active and in good standing with the State of North Carolina * Graduate of an accredited program * Current CPR/BLS certification * Valid Driver’s License Preferred: * One (1) year of clinical experience in a primary care, community health, or outpatient setting * Experience with underserved or rural populations * Familiarity with team-based care or PCMH/PCHH models WHY JOIN COMMWELL HEALTH?
