Lead and support nursing team members on your assigned neighborhood and shift. Observe, assess, and document resident conditions while ensuring families and physicians are kept informed. Oversee and implement individualized care plans from admission through ongoing updates. Foster a safe, positive, and relationship-centered environment for residents and staff. Coordinate appointments, transportation, and interdepartmental communication. Support team accountability, including initiating disciplinary procedures when needed. Qualifications: What You Bring Current LPN or RN license. Certified in CPR by an approved program that meets state and federal regulation. 1 year clinical experience preferred. Strong communication and interpersonal skills. Ability to stay organized and focused in a dynamic environment. Experience with EHR systems and basic technology tools (preferred).
