Summary: Vighter is recruiting Registered Nurse (RN) Case Managers to provide primary Case Management for Air Force Airman Guardian Medical Transition Unit patients hospitalized or lodging at Walter Reed National Military Medical Center (WRNMMC) Bethesda, MD. In accordance with established policies, procedures, protocols, and guidelines, while supporting the healthcare needs of Service Members, their families, and Veterans. Principal Accountabilities: * Provide case management, care coordination, and discharge/disposition planning for inpatient and outpatient care. * Identify and assess patients for case management, including referrals, Disability Evaluation System (DES) cases, and wounded, ill, or injured Service Members. * Develop and implement patient-specific care plans with patients, families/caregivers, providers, and the healthcare team. * Coordinate referrals and services across military, civilian, VA, rehabilitation, long-term, community, and home-care settings. * Address medical, nursing, and psychosocial needs while educating and advocating for patients and families/caregivers. * Monitor patient progress, treatment adherence, follow-up, outcomes, and resource utilization. * Maintain military and community resources, including MTF, TRICARE, National Guard/Reserve, VA, and community agencies. * Participate in multidisciplinary, Care Coordination, facility, and Andrews Medical Group meetings as required. * Complete medical record documentation, coding, tracking logs, data reporting, and required medical progress reports. * Follow applicable DoD, Air Force, MTF, CMSA, URAC, AAAHC, TJC, EEO, safety, and infection-control requirements. * Complete required electronic medical record, MTF orientation, AF/DoD Case Management, and other mandated training; support staff orientation and facility committees as required. * Support research, program evaluation, accreditation readiness, reports, and briefings. * Travel between medical treatment facilities or off-installation when required, including patient living-quarter assessments for safety and accessibility. * Coordinate leave, TDY, and paid time off with the COR, FRED, and Medical Management Staff, ensure patient-care coverage, and use Government-provided communication equipment during required travel. Knowledge and Skills: * Knowledge of case management assessment, planning, facilitation, advocacy, care coordination, and evaluation. * Knowledge of nursing practice, human behavior, family systems, and the impact of illness or disability on social functioning. * Working knowledge of DoD, Federal, State, and non-profit healthcare systems, including TRICARE, Medicare, Medicaid, VA, public health, and community services. * Knowledge of HIPAA, AAAHC, TJC, CMSA, and URAC standards and requirements. * Proficient with Microsoft Office, Teams, Outlook, internet applications, MHS GENESIS, Joint Legacy Viewer (JLV), and electronic documentation. * Strong communication, organization, critical-thinking, and problem-solving skills, including tact when working with distressed or uncooperative individuals. Education: * Graduate of an associate degree in nursing (ADN) or baccalaureate degree in nursing (BSN) program accredited by a national nursing accrediting agency recognized by the U.S. Department of Education. Qualifications: * Minimum of five (5) years of nursing experience, including three (3) years for ADN or two (2) years for BSN of full-time clinical case management experience. * Maintain an active, valid, current, and unrestricted RN license in any U.S. state or jurisdiction. * Current BLCS/BLS and Advanced Cardiac Life Support (ACLS) certification. * Clinical case management experience must include work with adults, children, families, seniors, or groups. * CCM or ANCC certification, or certification eligible; if not currently board-certified, obtain certification within 12 months of initial employment. * Must be a U.S. Citizen Duty Hours / On-call Requirements:  * The expected duty hours for this position are Monday – Friday, 0730-1630, with 1 hour break as stated within the contract.  Work Environment: * Location: Walter Reed National Military Medical Center, Bethesda, MD * Supportive team of healthcare professionals and administrative staff. * Opportunity to work with diverse populations, including Service Members, Veterans, retirees and their families.
