Job DetailsJob Location: JHCC-SNF - MILWAUKEE, WI 53202Job Shift: FLEXJob Purpose: - The Registered Nurse at OVATION COMMUNITIES is responsible for providing high-quality nursing care to residents, ensuring their health and well-being. This role involves assessing patient needs, developing personalized care plans, and collaborating with a multidisciplinary team to deliver compassionate and effective healthcare services. Key Responsibilities: - Conduct comprehensive assessments of residents' health conditions and develop individualized care plans. - Administer medications and treatments as prescribed, ensuring adherence to established protocols. - Monitor and document residents' progress, reporting any changes in condition to the healthcare team. - Collaborate with physicians, therapists, and other healthcare professionals to coordinate care and optimize patient outcomes. - Educate residents and their families on health management and disease prevention. - Maintain accurate and up-to-date medical records in compliance with regulatory standards. - Participate in quality improvement initiatives to enhance the delivery of nursing care. - Provide emotional support and guidance to residents and their families, fostering a supportive and caring environment. - Ensure adherence to infection control and safety protocols to protect residents and staff. - Supervise and mentor nursing assistants and support staff, promoting a culture of teamwork and continuous learning.Qualifications Required Education: - Bachelor of Science in Nursing (BSN) from an accredited institution - Current Registered Nurse (RN) license in the state of practice Required Experience: - Minimum of 2 years of clinical nursing experience in a healthcare setting - Experience working with elderly or geriatric patients preferred - Proven track record of managing patient care plans and collaborating with interdisciplinary teams Required Skills and Abilities: - Strong clinical assessment and critical thinking skills - Excellent communication and interpersonal skills - Ability to work effectively in a team-oriented environment - Proficiency in electronic health records (EHR) systems - Compassionate and patient-centered approach to care - Ability to handle stressful situations and adapt to changing environments
