This role includes conducting health assessments, assisting with care planning such as scheduling and accompanying appointments, and performing skilled nursing procedures to ensure each client's medical needs are safely and effectively managed. The candidate should have a compassionate and patient-focused approach and use their extensive medical knowledge and skills to ensure the well-being and comfort of residents. Their duties and responsibilities include, but are not limited to: Conducting initial assessments of new residents’ health condition Creating personalized care plans in conjunction with the L/ALD, doctors, other nurses, and care professionals Administering medication and treatments as prescribed by physicians Monitoring vital signs and observing the condition of residents closely Providing emotional support and counseling to residents and their families Communicating and collaborating effectively with residents, their families, unlicensed staff, case managers, healthcare professionals, and the management team Maintaining accurate records of residents’ health information Responding promptly to any changes in health condition or emergency situations Ensuring that the residential facility is clean and safe for residents Providing education to residents and their families on how to manage their illness or injury Providing training and ensuring competency of all unlicensed staff Ensure compliance with Minnesota Department of Health regulations, as applicable Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans Administer medications and monitor patients for side effects or reactions Document any changes in patient conditions and report on changes Maintain accurate records of patient care and discuss observations with the supervisor Preferred Qualifications Experience with medication management Experience with a geriatric population Experience with Assisted living, memory care, or long-term care setting Experience with mental health and managing behaviors Additional Qualifications Currently licensed Registered Nurse (RN) in state of employment Previous experience as a Registered Nurse is preferred Excellent interpersonal and client care skills Valid driver’s license First aid/CPR certification required Flexible work from home options available.
