Develops nursing care plan in coordination with patient, family and interdisciplinary staff as necessary. Communicates changes in patient’s clinical condition with Physicians, Nursing Supervisor/Manager, and co-workers as appropriate. Participates in discharges planning process. * Deliver direct and indirect patient care using the nursing process—Assessment, Planning, Intervention, and Evaluation  * Collaborate closely with patients, families, and our interdisciplinary care team to create personalized care plans  * Monitor and communicate changes in patient conditions to physicians, supervisors, and colleagues  * Play a key role in the discharge planning process to support safe, successful transitions    Why join us We believe that investing in our employees is the first step to providing excellent patient care.
