Overview Rapidly growing! Urgently hiring! $1,500 sign-on bonus!!!* LPN/RN Schedule– Monday-Thursday 9a-5p. Additional hours may be available. Homecare with Heart is offering you the most competitive pay scale in the area. We're looking for compassionate and experienced nurses to provide care for our clients in their homes. As a Home Care Nurse, you will be responsible for overseeing the quality, expert care of your assigned homecare patients. Responsibilities What You’ll Do: ∙Administer medication in accordance with standards of practice. ∙ Encourage social\rehabilitative skills and encourage self-care and independence. ∙ Perform client treatments in accordance with Plan of Care ∙ Assist the consumer in learning appropriate self-care techniques. ∙ Prepare clinical and progress notes to document care provided. ∙ Monitor, document accordingly and report any change in client condition to Clinical Director/Clinical Supervisor ∙ Promote activities to achieve a safe and healthy environment. ∙ Observe aseptic technique when performing client procedures/care. ∙ Collaborate with other members of the clinical team to develop and improve the Plan of Care ∙ Assist the client in achieving established goals in the Plan of Care developed by the RN. ∙ Maintain productivity according to the standards of the agency. ∙ Maintain license, CEUs and mandatory employment requirements. ∙ Participate in client care meetings, in-services, orientation and staff meetings. ∙ Provide service in accordance with agency’s philosophy, standards, policies and scope of practice ∙ Participates in Performance Improvement and adheres to HIPAA guidelines. ∙ Other duties as assigned. Annual MUI (Incident) training as dictated by the State of Ohio Medicaid, Ohio Home Care Program and Department of Developmental Disabilities Must be able to read and understand various documents used by the agency including Plan of Care, flow charts, client charts, agency policies and procedures. Must be able to use a computer, laptop or tablet. Must be able to think independently, recognize potential problems, gather information, and know when to contact a supervisor.
