Position Title Registered Nurse (RN) Care Navigator Job Description Purpose & Scope: Supports patients across the continuum of care by coordinating transitions of care, conducting patient outreach, supporting quality and care gap closure, and facilitating effective care coordination within the ambulatory setting between patients, providers, and post‑acute settings. Functions within a multidisciplinary care team to improve patient experience, clinical outcomes, and performance on quality and population health initiatives. Education: Associate’s Degree in Nursing required. Bachelor’s degree in Nursing preferred. Experience: 3 years of healthcare, or nursing/patient care experience is required. Prior experience as a Nurse Navigator in transitions of care, care coordination, outreach, ambulatory care, population health, and/or quality program support preferred. Certification/Licensure: Licensed as a Registered Nurse by the Virginia Department of Health Professions or compact state licensure is required.
