We're growing and expanding our service areas, and we're looking for multiple RNs to join our Home Health team, with a current focus on Buckley, Puyallup, and Graham. What You'll Do As a Home Health RN, you'll have the opportunity to build meaningful relationships with patients while providing skilled care where they're most comfortable: at home. Assess patients in their homes and identify their individual care needs Develop and manage personalized plans of care in collaboration with providers Provide skilled nursing care, education, and support to patients and families Coordinate care with therapists, physicians, and other members of the interdisciplinary team Monitor patient progress and adjust care as needs change Complete accurate, timely clinical documentation Participate in quality initiatives that help us continually improve patient care What You'll Bring Current Registered Nurse (RN) license Current AHA BLS certification Home Health and/or Hospice experience preferred Valid driver's license, insured vehicle, and reliable transportation Ability to work independently while collaborating with a clinical team Our Core Values: CAPLICO Customer Second (Employee First!) • Accountability • Passion for Learning • Love One Another • Intelligent Risk Taking • Celebration • Ownership Want to learn more?
