Full Job Description

Registered Nurse (RN) – Long-Term Care Floor Nurse Provide safe, compassionate, and individualized nursing care to residents in accordance with physician orders, nursing standards, and facility policies. Assess residents upon admission and routinely thereafter, including monitoring vital signs, pain, skin integrity, nutritional status, and changes in condition. Administer medications and treatments safely and accurately, following the Five Rights of Medication Administration and applicable Texas regulations. Monitor residents for changes in condition and promptly notify the physician, nurse practitioner, supervisor, and/or family as appropriate. Perform and document nursing assessments, progress notes, medication administration, treatments, and other required clinical documentation. Provide wound care, catheter care, infection-control measures, and other skilled nursing treatments as ordered. Develop, implement, and evaluate individualized resident care plans in collaboration with the interdisciplinary team. Supervise and provide direction to LVNs/LPNs, CNAs, and other nursing staff within the RN's scope of practice. Delegate appropriate nursing tasks and follow up to ensure that resident care is completed safely and effectively. Respond appropriately to emergencies, falls, acute changes in condition, and other clinical situations. Communicate effectively with residents, families, physicians, pharmacy personnel, therapists, and other members of the healthcare team. Participate in physician rounds, care conferences, quality-assurance activities, and discharge planning as assigned. Maintain accurate and confidential medical records and ensure documentation is completed in a timely manner. Follow federal and Texas regulations, including infection prevention, resident rights, medication management, and long-term-care requirements. Maintain a clean, safe, and respectful environment and advocate for residents' dignity, safety, and quality of life. Report incidents, accidents, medication errors, suspected abuse/neglect, and significant changes in resident condition according to facility policy and regulatory requirements. Provide education to residents, families, and caregivers regarding medications, treatments, disease management, and health needs. Maintain current RN licensure and required certifications and participate in continuing education and facility training. Core responsibilities: Resident assessment, medication administration, skilled nursing care, clinical documentation, care-plan management, staff supervision/delegation, physician communication, emergency response, infection prevention, and resident advocacy.
