Principle Duties: * Admits new clients to private duty services and re-evaluates client needs. * Develops the plan of care. * Performs skilled nursing services as requested by the client and as prescribed by the healthcare practitioner. * Communicates with the physician regarding the client’s needs and reports any changes in the client’s condition; obtains/receives physician’s orders as required. * Communicates with community staff to strategize about how to improve client outcomes. * Performs supervisory visits to evaluate client’s satisfaction with care and services provided. Qualifications: * Graduate of an accredited school of nursing. * One (1) to two (2) years of recent acute care or home care experience preferred but not required. * Current RN/LPN licensure in the state. * Current CPR certification. * Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. * Has the ability to read, follow written instructions, and document services provided. * Is self-directed with the ability to work with little direct supervision. * Is kind, reliable, and compassionate.
