Job Summary The Outcomes Analyst reviews medical record documentation and provides education to clinicians to meet requirements of mandatory quality programs such as ScionHealth’s National Quality Program and Centers for Medicare and Medicaid Services (CMS) Quality Reporting Programs for the ScionHealth Hospital Division. This position will assist in the ongoing monitoring of identified outcome benchmarks for the inpatient population. This position will improve the quality and accuracy of documentation standards by auditing, analyzing data, educating, and supporting the interdisciplinary team members on appropriate documentation and other drivers of quality outcomes. The Outcomes Analyst has access to confidential patient information and maintains all confidentiality of all information. Essential Functions Quality Outcomes Serve as subject matter expert for CMS Quality Reporting Program for all Specialty facilities. Stays current with how data interfaces between Hospital Division electronic systems and provides education as needed. Serves as a resource for Clinical Informatics team for needed updates/changes, testing and deletions of EMR. They will also serve as SME for any needed responses to EMR update questions. Monitors and reports QRP thresholds. Assist facilities not meeting threshold with analysis of data, action plans and follow up until compliant. Monitors the CMS Quality Reporting Program website for reporting announcements and FAQ updates. Acts as a backup submitter for the transmission of CARE Data Set records into the CMS national database upon request and approval from Senior Director, Quality Reporting and Outcomes. Completes chart audits and monitoring of documentation outputs from the medical record and other systems to ensure documentation compliance with quality standards of both CMS Quality Reporting Program and the ScionHealth National Quality Strategy as assigned. Acts as a resource for Director of Quality in conducting CARE Data Set record data integrity audits through comparison of medical record documentation to the LTRAX documentation. Education Collaborate with hospital leaders to provide education support onsite in quality reporting or patient safety related documentation education to staff for facilities identified in quality reporting crisis. Creates tools and educates on CMS Quality Reporting Program Other Conducts job responsibilities in accordance with the standards set out in the Company’s Code of Business Conduct, its policies and procedures, the Corporate Compliance Agreement, applicable federal and state laws, and applicable professional standards. Serves on Support Center committees upon request and approval from Senior Director Quality Reporting and Outcomes. Other duties as assigned.
