Job DetailsJob Location: Horizon Home Healthcare LLC - Vandalia, OH 45377Registered Nurse (RN) - Home Health Location: Montgomery County, Ohio and surrounding areas Employment Type: Full-Time | Part-Time | PRN Deliver Exceptional Care Where Patients Feel Most Comfortable We are seeking a compassionate and experienced RN to join our home health team. In this role, you will manage patient care from admission through discharge, complete OASIS assessment, develop individualized care plans, and collaborate with an interdisciplinary team to achieve the best possible outcomes. Responsibilities Performs Start of Care (SOC), Resumptions of Care (ROC), Recertification, Transfer, and Discharge OASIS assessments Conducts comprehensive nursing assessments and develop individualized plans of care Coordinates and manages patient care throughout the episode of care Provides skilled nursing services, patient education, medication management, and disease management Collaborates with physicians, therapists, social workers and other members of the interdisciplinary team Monitors patient progress and adjust care plans as needed Completes accurate and timely OASIS documentation and clinical records in accordance with Medicare, state and agency guidelines Participates in quality improvements initiatives and maintains compliance with regulatory standards Qualifications Current Ohio RN license Experience in home health and OASIS assessments preferred, but not required Knowledge of Medicare home health regulations and documentation requirements preferred, but not required Current CPR and/or BLS certification Valid driver's license, reliable transportation, and proof of auto insurance Strong assessment, communication, organizational and time management skills What We Offer Weekly pay Pay per visit Paid time off Medical, vision and dental benefits NO ON-CALL rotation Sign on bonus Weekly gas incentives (up to $500 a month) Continuing education Supportive clinical leadership and interdisciplinary team Manageable caseloads Technology and documentation support Why Join Our Team?
