GiaCare Inc. is accepting applications from experienced Disease Managers (RN) for a future opportunity supporting MacDill Air Force Base at Tampa, Florida. This position is being recruited in anticipation of contract award. Qualified candidates will be considered for employment upon contract award. ABOUT THE OPPORTUNITY Join the 6 MDG healthcare team as a Disease Management RN, supporting active-duty service members, retirees, and their families through proactive chronic disease management, patient education, and population health coordination. SCHEDULE * Full-Time * Monday–Friday * 9-hour shifts, duty hours 0730–1630 * 60-minute unpaid lunch * Not to exceed 40 hours per week * No on-call RESPONSIBILITIES * Apply the nursing process across all patient age groups in a disease management capacity * Coordinate care for patients managing chronic conditions (e.g., diabetes, hypertension, COPD, obesity) * Provide patient education and self-management coaching to support long-term health outcomes * Deliver services in both in-person and virtual/telehealth formats * Monitor patient progress and adjust care coordination plans in collaboration with the care team * Document patient encounters accurately within the electronic medical record * Complete the Disease Management Formal Training Course (DMFTC) within 30 days to 4 months of starting on the task order MINIMUM QUALIFICATIONS * U.S. Citizen, U.S. national, lawful permanent resident, or individual otherwise authorized to work in the United States * Associate's Degree of Nursing * Current, full, active, unrestricted Registered Nurse license in any U.S. state, territory, or the District of Columbia * Minimum 1 year of primary/outpatient ambulatory care and telehealth experience as an RN within the past 2 years * Current Basic Life Support (BLS) certification (or willing to obtain) * Able to pass a Tier 1 (or higher) background investigation for facility and network access PREFERRED QUALIFICATIONS * Prior experience in disease management, chronic care coordination, or population health programs * Experience with military or federal healthcare beneficiary populations * Strong patient education and telehealth communication skills WHY GIACARE?
