Job DetailsJob Location: Main Office - Columbus, OH 43215Position Type: ContractorEducation Level: High SchoolJob Shift: AnyJob Category: NurseQualificationsRequirements and Onboarding Checklist To be cleared for contract activation, the RN must meet the following rigid criteria and provide physical copies of all credentials to be filed in Tab 5 (Contracted RN Portfolio): • Active Ohio RN Licensure: A current, unencumbered Registered Nurse (RN) license in good standing with the Ohio Board of Nursing (verified via a dated e-License center printout). • Professional Experience: Minimum of 2 years of clinical nursing experience (Prior experience working within ODA PASSPORT, AAA, or DODD waiver lines is highly preferred). • Emergency Medical Readiness: A valid, hands-on CPR and First Aid certification card from an approved provider (e.g., American Heart Association or American Red Cross). • Insurance Coverage: An active individual Professional Liability (Malpractice) Insurance policy certificate showing minimum limits of $1,000,000 per occurrence / $3,000,000 aggregate. (Note: applicable for monthly premium reimbursement) • Pre-Contract 7-Database Registry Check: Signed consent and verified "Clear" status across the 7 mandatory state registries (SAM, OIG, Nurse Aide, Sex Offender, DODD Abuser, Medicaid Exclusion, BCI Retained Fingerprints) run before the contract start date. • Electronic Fingerprint Clearances: Completed BCI (and FBI, if living outside Ohio in the past 5 years) electronic fingerprint checks submitted specifically under ODA reason codes and mailed directly to the ODA Columbus headquarters. • Administrative Service Agreements: A fully executed Professional Services Contract, a completed IRS W-9 form, and a signed copy of the agency’s HIPAA Cloud & Digital Data Security Policy.
