HOW YOU'LL MAKE AN IMPACT Your Role: Performs Minimum Data Set (MDS), case management, utilization review, quality assurance (QA), and discharge planning duties. Conducts federally mandated patient assessments based on Centers for Medicare and Medicaid Services (CMS) guidelines, including appropriate care planning, interdisciplinary communication, and Agency for Health Care Administration (AHCA) documentation preparation. Promotes wellness. Is responsible for collecting integral data and compiling/transmitting data to CMS. Determine patients’ needs based upon illness, diagnosis, prognosis, and social support systems. Contacts appropriate agencies to coordinate patient discharges into the community or long-term care settings. Ensures facility compliance with applicable state and federal regulation. Collaborates with other members of the healthcare team. Reports To: Administrator, Rehabilitation Center (JC: 013501) [with delegation (as needed) to RN/Director, Rehabilitation (JC: 030101)] Supervises: None WHAT YOU’LL NEED TO APPLY Required Education: Associate degree in nursing. Preferred Education: Bachelor’s degree in nursing. Required Experience: One (1) year of relevant work experience. Preferred Experience: Two (2) or more years of relevant work experience Required Certification/License/Registry: Florida RN license.
