About Us: Stratford Villa Post-Acute is proud to be a Five-Star, Medicare-certified, 37-bed Skilled Nursing Facility located near the heart of downtown Livermore. It is part of the Villas Post-Acute and Senior Living community for seniors in the heart of Livermore. Position Overview: The Registered Nurse (RN) supervises and coordinates the efforts of nursing staff and provides total nursing care for residents. In addition to the essential functions outlined herein, this position is considered to be a direct resident care role and therefore the incumbent shall meet and fulfill all applicable requirements as outlined in California Code of Regulation Title 22: Social Security, Division 5: Health Facilities, as well as the Health and Safety Code for the State of California. * Conduct thorough assessments of residents' health status and needs. * Develop and implement individualized care plans based on assessments. * Administer medications and treatments as prescribed by physicians. * Monitor residents' progress and make necessary adjustments to care plans. * Provide education to residents and families about health conditions and care options. * Collaborate effectively with interdisciplinary teams to ensure optimal care. * Document all aspects of patient care accurately and timely in electronic records.
